2007 SOCCER

Prospective Soccer Players and Parents:

Tryouts for the 2007 boys' and girls' soccer teams at Gotha Middle School will begin on Monday, August 20th. Tryouts will be held each day after school from 4:00 p.m. until 5:45 p.m. (except Wednesdays, which will be from 3:00 - 4:45). Please arrange to be picked up no later than 6:00 (5:00 on Wednesdays). The final team will be announced on Monday, August 27th. Our first game is tentatively scheduled for Wednesday, Sept. 6th.

The following items are required for a child to participate in tryouts:

- OCPS Sports Screening Physical Exam Form filled out & signed completely.

(Please make sure the physician has signed and stamped the form and that the parent/guardian and athlete have signed on the front page . If you do not have personal insurance on the athlete, you must write “none” under verification of insurance (page 3) and sign your name next to this area.

- Risk form (initials required, do not just check; signature and initials required at end).

- EMT card (emergency treatment card)

- Addendum to Athletic Sports Screening Form

- 2.0 cumulative GPA or above on a 4.0 scale or a 2.0 GPA or above in the previous semester. (Except incoming 6 th graders).

Note: Students may participate in athletics only once each grade level.

Please be sure to read the forms carefully and make sure they are filled out completely and signed in all places. A student will not be allowed to tryout with incomplete paperwork!

The decision on who makes the soccer team will be based on the following criteria:

1. Grades
2. Attitude
3. Behavior (at practice and in school)
4. Leadership
5. Ability to listen and follow directions
6. Effort and potential
7. Skill and ability
