Student Medication If your child is taking medication that is to be administered at school, it MUST be kept in its original container, with a label affixed by the physician or pharmacist, showing the child's name, the dosage, and the schedule for administration. The parent or guardian must complete an "Authorization for Medication" form for all medication that will be distributed by Gotha personnel. The number of pills or tablets brought to school will be counted by the nurse and the person bringing the medication. The count will be noted on the form. When medication is dispensed, the student will be asked to sign a log noting date, time, and dosage. It is the student's responsibility to remember his or her medication schedule. Students are not permitted to carry medications with them whether they are prescription or over-the-counter drugs. The only exception is for students who must carry inhalers because of Asthma. |
